Multiple Sertoli Cell Adenomas in Testicular Feminization - A Case Report -



Abstract
The testicular feminization is a type of male pseudohermaphroditism occuring in genetic male with well developed female secondary sex characteristics. It often accompanies Sertolicell adenoma of immature tubules, mostly incidental and minute, in
the
undescended testis. Furthermore, multiple development of Sertoli cell adenomas with evolutional change by increase of tumor size is very unusual.
A 21-year-old phenotypically female patient consulted a gynecologist because of amenorrhea. The patient had normal external genitalia of the female type and a normal vagina. Pubic and axillary hair was nonexistent (Tanner stage I). The breasts
were
large and pendulous(Tanner stage V). Blood chromosomal study revealed a 46, Xykaryotype. Both undescended testes, 2.5 cm and 2.7cm each in length, were removed from both inguinal canals and a total of eleven discrete, light tan, firm nodules
ranging
from 0.2 to 1.5 cm were found in both gonads. They were composed of uniform tubules of well differentiated Sertolic cells, similar to tubules seen in infantile testes. Dense collagenous bundles incorporated in the larger nodules together with
participation of Leydig cells suggest a hamartomatous growth of this multiple nodules.
